Civil Registration is a continuous, permanent, compulsory recording of vital events occurring in the life of an individual such as birth, marriage, and death, as well as all court decrees, and legal instruments affecting his civil status in appropriate registers as mandated by Act No. 3753, the Civil Registry Law. |

For those who were issued a negative result
1. Request a copy of your document from the local civil registrar’s office (LCRO) of your place of birth, marriage or death. 2. If your requested document is available, request the local civil registrar (LCR to endorse a certified true copy of NSO-OCRG. If the document is no longer available but the entries are available in the registry book, request LCR to issue a transcription (LCR Form 1A, 2A or 3A).On the portion “This certification is issued to __________…” the correct entry should be “NSO-OCRG”. The endorsement must be addressed to:

3.Bring the following papers to NSO, Ground Floor, Vibal Building and submit to Endorsement (Area A) in order to get a copy of the endorsed document: 1. Receipt of forwarder/carrier (JRS/LBC/others); 2. Personal copy of the document; 3. Endorsement letter or transmittal letter from the LCRO; 4. Negative certification (intact, partially destroyed, destroyed or not available) from the NSO. A claim stub will be issued with the date and time of release of the document Releasing will be at Area B. 4. If requested document is not available at the LCRO, apply for late registration. Request LCR to endorse the document following steps 2 to 3. |
